Secondary Rhinoplasty to Improve Nasal Tip Definition, Projection, and Rotation Without Revision of Previous Grafts: The Keim Technique

Abstract

Introduction: Loss of rotation and projection after rhinoplasty not only results in a deformed tip, but can also create a supratip deformity, requiring revision surgery (secondary rhinoplasty) for correction. 

Objectives: To describe the Keim Technique, a novel approach to secondary rhinoplasty which allows improvement of nasal tip definition, projection, and rotation without redoing primary grafts. 

Methods: Qualitative, descriptive, observational, retrospective study. Data were obtained from the electronic medical records of patients treated in private practice by the first author. 

Results: The study cohort consisted of six female patients who had already undergone primary rhinoplasty but wanted a more delicate and better-defined nasal tip. All underwent secondary rhinoplasty with an open approach using the Keim Technique; the average follow-up time was 18 months. 

Conclusion: Secondary rhinoplasty is a safe surgery with a very low rate of infectious complications. The Keim Technique is a viable option with good short- and medium-term outcomes and excellent patient satisfaction, but it must be performed by an experienced surgeon.

Biography

Frederico Keim is an otorhinolaryngologist and specialist in facial plastic surgery. He has performed more than 3,000 rhinoplasties and is a reference in structured rhinoplasty in Brazil, sought after by people who need not only aesthetics surgery, but also reconstructive surgery.
 
Júlia Gaspar is an Otorhinolaryngologist with a Medical Residency completed at the Federal University of the State of Rio de Janeiro (UNIRIO); Master in Medical Sciences at the Federal University of São Paulo (UNIFESP), did a Fellowship in Facial Plastic Surgery at the IPO Hospital (Paraná Institute of Otorhinolaryngology); and has a Postgraduate degree in Structural Rhinoplasty from the Jurado Institute.
